Support Coordination

Support Coordination is designed to support participants and families to get the best outcomes from their NDIS Plan. The Brightside Healthcare Support Coordination team works in consultation with participants to effectively maximise the usage of their NDIS funding by advising them of the flexibility available within their plans and by connecting them to a reputable range of service providers based on individual needs and life goals.
As a Participant being supported by Brightside Healthcare Support Coordinators you will be able to:
- Upskill and educate yourself on the NDIS and how to better understand and navigate your plan, further empowering yourself
- Understand how you can better use your NDIS plan to achieve your goals;
- Manage all your available resources more effectively to get the best possible outcome;
- Work with people in your support circle to meet the goals of your NDIS plan;
- Teach you how to use the NDIS portal so you can check and monitor the spending of your funds;
- Monitor the progress of your goals;
- Help you prepare for your next NDIS review appointment
- Identify housing solutions for Supported Independent Living
- Receive support in the event of a crisis or change of circumstances
- Connect with disability and allied health service providers as well as mainstream organisations
- Build capacity to exercise choice and control
The Support Coordination process
Identify
Identify and present a range of suitable options for you in the NDIS marketplace.
Coordinate
Coordinate Service Agreements and liaise with service providers.
Track
Keep track of your plan to ensure your NDIS funds are used effectively.
Report
Provide the NDIA progress reports and prepare for your plan review meetings.
Understanding Support Coordination
A Support Coordinator will help to connect you with all the individual services outlined in your plan, helping you to get the best out of your NDIS funding.
A support that enables you to build your independence skills and capacity so as to reduce reliance on funded supports.
Case Management is similar to Support Coordination in that it assists you to link with providers. Support Coordination is different to Case Management in that it is more about building your capacity, supporting you to become more independent.
The LAC program is a time limited support that works with the NDIA to build your plan and to start you on your NDIS journey.
The Support Coordination role is a funded support that will appear on your plan as a Capacity Building Support. The amount of time your coordinator has to work with you will vary for each participant, depending on the budgeted amount. Your support coordinator will work more closely with you to assist you to use your plan to meet your needs and to navigate the NDIS systems.
A Plan Manager is responsible for the financial/administrative side of a participant’s NDIS Plan. For example, the Plan Manager is responsible for organising payments, processing claims and invoices and tracking budgets. If you require plan management services, your Support Coordinator can help you to arrange this.
